For Alumni to Request a Copy of Personal CHOC Records:

The forms that follow are to help you request information about you, your family and your own medical records.  Please know that in most cases in the years before 1960 the only information we took was on the intake form, and sometimes a birth certificate or other announcements were added to the file. We know that your search for information can sometimes be very time consuming.  We will do our best to respond quickly.  If you have questions please call us.

In 2007, May Zellers Preble (1955-1971) initiated an effort to reconnect with former CHOC residents and formed the CHOCA (CHOC Alumni) Committee.  Her main purpose in doing this started with her own desire to reconnect with the people who were her childhood friends.  The Group hopes to reconnect former residents who would like to gather with folks that were at The Children's Home in their early years.  The committee is headed up by May Preble and is supported by the Development Office staff, Cindy Bartholomew and Sharon Graves.  Your information will be handled in the manner you request, and if you do not wish to receive regular mail or e-mail from CHOC, please state that.
